Parallel Perception

I acknowledge that I have voluntarily chosen to participate in the courses offered by Lujan Matus. I have full knowledge of the risks that these activities present, including travel to, participation in, and returning from the activities or events.

I understand that by being permitted to participate in shamanic tuition, and martial arts activities, I agree to assume any and all risks of injury or death. I further understand and agree to assume responsibility for all risk of theft, loss, or damage of personal property which occurs at any time arising out of my participation in activities with Lujan Matus.

I understand and agree that as a condition of participation in activities and tuition, I further agree to release liability and indemnify Lujan Matus, the officers, directors, agents, employees, assigns, successors, or lessors from any damage, injury, or death to myself or to any person or property, in any way connected with my participation, and tuition received from Lujan Matus.

I understand and agree that I have carefully read this agreement and fully understand all of its terms and conditions. I understand that this is a release of liability which could legally prevent me from filing suit or making any other claim for damages in the event of my death or injury to me. With this knowledge, I am entering into this agreement freely and voluntarily.

I understand that I should consult a physician prior to beginning any activity program, including shamanic movement. I recognize that it is my responsibility to notify Lujan Matus of any serious illness or injury before every class.

I understand that shamanic movement consists of physical activity, and that injuries may occur during the normal course of class activities.

I agree that all movements learned during the course are for personal use only and are not to be demonstrated or taught to others unless Lujan Matus has given expressed permission.

I agree that this is binding on myself, my assigns, and legal representatives. I understand and agree that I have read all of this waiver and release.

Cancellation

If the participant cancels their tuition or is unable to attend, under any circumstances, refunds are not available. Tuition fees may be transferred to a future date or utilized for other activities ( including online tuition ) that are provided by Lujan Matus if at least two months notice is provided.

Lujan Matus reserves the right to decline to accept or retain any person whose actions, physical health, mental condition or general deportment impede trip operation or rights, welfare or enjoyment of other workshop members.
